NHS GP registration has changed and you are now required to register online.
To find out if you are living in the catchment area for this practice please visit the NHS find a GP service.

If you are eligible to register with the practice please use this link to complete a registration form.

Please identify if you speak and write in English when you fill in this form. If you have another preferred language please inform us. 

If you are registered with a pharmacy at your previous practice you must change this to a local or preferred pharmacy. If you have a repeat prescription slip please hand it to the reception at the Parks Medical Centre.

Homeless Patients/Patients without ID

If you are currently homeless you are still able to register with a GP.

You can give a temporary address, such as a friend’s address, a day centre or the GP surgery address. Please speak to reception if you have any questions.

You do not need proof of ID to register with a GP, but it might help if you have one or more of the following:

• passport
• birth certificate
• HC2 certificate
• rough sleepers’ identity badge
• hostel or accommodation registration or mail forwarding letter

Online Consultations

PLEASE READ THE STATEMENT BELOW BEFORE YOU USE THIS SERVICE.

Our online consultation system is used to request medical advice, extend existing sick notes (not new ones), send photographs (not to include any identifiable or sensitive features) or to ask an admin question.

Please DO NOT use this service for urgent medical problems or medication requests. To order your repeat medication please register for online services. You can register by contacting the surgery by telephone or in person.

The service is monitored daily and we endeavour to respond on the same day. However, requests after 3pm may be dealt with on the next working day.

Please note, this service is NOT monitored on weekends and bank holidays.
